Amending Your Application
Researchers will need to make an amendment request if they need to change the sampling, recruitment strategies, research design, add team members (e.g. transcriptionist), adapt data collection instruments, or need to change when/how the researchers contact research participants or use participant data beyond what was outlined in the Informed Consent Form.
In qualitative research, elements of research design often emerge during the research process. Unless these elements change the level of risk or have other ethical implications, researchers do not need to submit them as Amendments.
All amendments must be related to the original application. Substantive changes may require a completely new application.
To Request An Amendment:
- Using track changes in Word and with approval from your supervisor (if you are a student), amend the approved REB Application. Remember to also revise the Appendices .
- Rename your Application to: [date][last name][first initial]Amended REB Application (eg 2016_01_01 Smith T Amended REB Application.docx
- Download and complete the Amendment Request Form, which can be found on the REB FORMS page.
- Send your Amended REB application and Amendment Request Form to the REB member who approved the application cc your supervisor (if you are a student) and vanREBadmin@adler.edu.
- The subject line of the email should be: [Last Name][First Initial] REB Application AMENDMENT.
Renewing Your Application
Once your application has been approved, you will be granted permission to conduct your research for one year.
If your research continues past this one-year date, you will need to:
- Download and complete an Annual Status Report, which can be found on the REB FORMS page. This will extend your permission to conduct research for another year.
- Email your Annual Status Report to vanREBadmin@adler.edu and cc your supervisor (if you are a student)
- The subject line of your email should be: [Last Name] [First Initial] Annual Status Report.
Closing your Research Project
Once your research is complete or your contact with participants has ended, you must:
- Download and complete a Project Closure Report, which can be found on the REB FORMS page.
- Email your Project Closure Report to vanREBadmin@adler.edu and cc your supervisor (if you are a student).
- The subject line of your email should be: [Last Name] [First Initial] Project Closure Report
This form MUST be on file in order to graduate.
